Placebos and treatment of pain.

چکیده
A 52-year-old female patient, with a long history of chronic low back pain treated over time with various modalities including, recently, opioids, goes on Friday evening to the emergency room for, she says, “excruciating, paralyzing, back pain.” The house officer working her up finds that she just ran out of the pain medications she was previously on; his evaluation is totally negative; the patient requests IV meperidine for her pain. The house officer agrees verbally to the patient’s request, but, instead of injecting meperidine, gives her a 2-mL syringe of normal saline IV “to see if it will work.” Is this treatment acceptable?

some experimental intervention. Although the most frequently used placebo is the 'sugar pill' in drug trials, placebos can be and have been used for all kinds of interventions, ranging from placebo ultrasound in the treatment of pressure ulcers and placebo surgery in the treatment of osteoarthritis to sham traction in the treatment of low back pain and placebo oestrogen implants in the preventi...
متن کاملRelieving pain using dose-extending placebos: a scoping review.
Placebos are often used by clinicians, usually deceptively and with little rationale or evidence of benefit, making their use ethically problematic. In contrast with their typical current use, a provocative line of research suggests that placebos can be intentionally exploited to extend analgesic therapeutic effects. Is it possible to extend the effects of drug treatments by interspersing place...

The American Society for Pain Management Nursing (ASPMN) holds the position that a placebo should not be used by any method to assess and/or manage an individual's pain regardless of their age or diagnosis. The only justifiable use of placebos is for participants enrolled in a blinded clinical trial. These clinical trials must be Institutional Review Board (or equivalent) approved with particip...

INTRODUCTION Patients require an accurate knowledge about placebos and their possible effects to ensure consent for placebo-controlled clinical trials is adequately informed. However, few previous studies have explored patients' baseline (ie, pretrial recruitment) levels of understanding and knowledge about placebos. The present online survey aimed to assess knowledge about placebos among patie...
متن کاملDifferent Placebos, Different Mechanisms, Different Outcomes: Lessons for Clinical Trials
Clinical trials use placebos with the assumption that they are inert, thus all placebos are considered to be equal. Here we show that this assumption is wrong and that different placebo procedures are associated to different therapeutic rituals which, in turn, trigger different mechanisms and produce different therapeutic outcomes. We studied high altitude, or hypobaric hypoxia, headache, in wh...
